Privacy policy

What data we process when you use klinikaokumed.pl or contact the clinic, and what rights you have.

1. Data controller

The controller of your personal data is OKUMED Spółka z ograniczoną odpowiedzialnością (OKUMED sp. z o.o.), registered at ul. Świętojańska 135/2, 81-404 Gdynia, Poland, tax ID (NIP) 5862419691, which operates the OKUMED clinic (premises on the 1st floor). Contact: phone +48 882 650 299.

2. What data we process and why

Contact data (name, phone number, e-mail address, message content) provided by phone or WhatsApp – to reply and arrange an appointment (Art. 6(1)(b) and (f) GDPR).

Medical data provided in connection with healthcare services – under Art. 9(2)(h) GDPR and Polish regulations on patient rights and medical records.

Technical data (IP address, browser type, pages visited) stored in server logs – to keep the website secure and working properly (Art. 6(1)(f) GDPR).

3. Online booking and messaging

Online booking takes place on the external service lekarznateraz.pl, and WhatsApp communication via WhatsApp (Meta Platforms Ireland Ltd.). By using these services you accept their terms and privacy policies. The Google map on the contact page loads only after you click; connection data is then sent to Google Ireland Ltd.

4. Cookies

The website does not use cookies for advertising or analytics. Only technical cookies necessary for the site to function may be stored. If analytics or marketing tools are introduced, you will be asked for consent.

5. Retention

Data from contact enquiries is kept for as long as needed to handle the matter, no longer than 12 months from the last contact, unless healthcare services are provided – in which case the retention periods for medical records apply (as a rule 20 years).

6. Recipients

Data may be entrusted to providers of IT, website hosting and e-mail services and, for medical records, to entities authorised by law. We do not sell personal data.

7. Your rights

You have the right to access, rectify, erase or restrict the processing of your data, the right to data portability, the right to object to processing based on legitimate interest, and the right to lodge a complaint with the Polish supervisory authority (Prezes Urzędu Ochrony Danych Osobowych, ul. Stawki 2, 00-193 Warsaw). Providing data is voluntary but necessary to reply or book a visit.
