Secondary cataract — why the laser used for capsulotomy matters with premium lenses
A few months or years after successful cataract surgery vision starts to deteriorate again — the image clouds over, glare appears and there is a sense of “fog”. Most often this is secondary cataract. It sounds worrying, but its removal is one of the quickest and safest procedures in ophthalmology. There is, however, one condition that few people talk about: which laser it is performed on — because with modern premium lenses this genuinely matters.
What actually is secondary cataract?
Despite the name, a cataract “does not come back”. During cataract surgery we remove the cloudy lens but leave its thin, transparent envelope — the capsule — in which the new lens (implant) is placed. In some patients the back wall of this capsule clouds and fibroses over time. This is posterior capsule opacification (PCO) — commonly called secondary cataract.
It is a natural and common phenomenon — affecting a significant proportion of patients after cataract surgery. It is not a surgical error or a sign that “something went wrong”.
Symptoms
- gradual deterioration of visual acuity,
- glare and reflections, especially at night and against the sun,
- a sense of looking “through fog” or a dirty window,
- reduced contrast.
The symptoms resemble the return of a cataract — hence the name.
Treatment: YAG laser capsulotomy
Secondary cataract is not treated with another operation. A YAG capsulotomy is enough — a procedure in which the laser creates a small, round window in the clouded capsule in the visual axis. Light once again reaches the retina freely, and visual acuity usually returns within a few hours.
The procedure is:
- painless and performed in the consulting room after anaesthetic drops,
- short — it takes a few minutes,
- one-off — once the capsule has been opened, the problem no longer applies,
- outpatient — the patient goes straight home.
Why the laser and the technique matter — especially with premium lenses
Here we come to the heart of the matter. Capsulotomy is performed just behind the lens implant. With an ordinary YAG laser there is a risk of two things:
- “Pitting” of the lens — micro-damage to the surface of the implant if the pulse lands too close to the lens.
- Floaters — particles in the vitreous that the patient sees as floating shadows.
With monofocal lenses small pits are often unnoticeable. But with premium lenses — multifocal, trifocal and EDOF — any damage to the precise optics can degrade their performance: reduce contrast, increase glare, disturb the splitting of light into focal points. These are lenses the patient has consciously paid for in order to enjoy vision without glasses — and their optics must be protected.
The Ellex laser with posterior offset and PROcap technology

What is the advantage? The laser allows the point at which the energy acts to be shifted up to 2000 µm (2 mm) behind the capsule — exploiting the properties of the cavitation bubble that forms. In practice this means that:
- the energy acts at a safe distance from the lens, which minimises the risk of pitting the implant — crucial with multifocal and EDOF lenses,
- the opening in the capsule is even and perfectly round (usually 4–5 mm),
- fewer iatrogenic floaters form in the vitreous,
- thanks to the efficient beam profile the procedure can be performed with lower energy, further reducing the risk of complications.
In other words: the same procedure, but performed in a way that protects what is most valuable in the patient’s eye — the precise optics of the premium lens and the comfort of vision.
Summary
Secondary cataract is a common and completely reversible problem — a few-minute laser capsulotomy restores sharp vision. It is worth remembering, though, that the quality of the equipment and the technique matter, especially when a premium lens is at work in the eye. Choosing a laser with posterior offset is genuine care for the durability of the result of the earlier operation.
